Home First Seminar
Hospital trusts and their community health and social care partners are being encouraged to adopt a “discharge-to-assess” (D2A) or “home-first” model involving patients who are medically fit for discharge and do not require an acute hospital bed, but may still require care at home or in a community setting. This prevents older people, who do not get enough opportunity to mobilise, from having an increased risk of reduced bone mass and muscle strength, reduced mobility, increased dependence, confusion and demotivation.
