Condition check guidance

In order to protect patient confidentiality we can only give out a condition check for a patient if we are given the name, date-of-birth and address of the patient and have the patient's or relative's permission to release the information.

In some situations anonymous condition checks can be given out for patients who have not been named in the public domain. However, we will still require a name, date-of-birth or address in order to verify and provide the information.

To make a request about a condition of a patient please call the York Hospital Switchboard on 01904 631313 and ask to be put through to the Bed Office.
