Advance Care Planning

Here at York Teaching Hospital NHS Foundation Trust, we care about what is important to you. 

Advance Care Planning is a process of thinking about and sharing your wishes for future care. It can help you tell others what would be important if you became ill and unable to communicate.

An Advance Care Plan is designed to help you plan for the future and record your preferences for the care you would or would not like to receive, and where you would like to receive that care. 

You do not have to complete a plan but it might help to reassure you that your wishes will be known when care is planned. A plan would help any care professionals looking after you to try to meet your wishes.
