Referral to the hospital palliative care service

To make a referral to the hospital palliative care service please contact 01904 725835 and the most appropriate healthcare professional will assess.

We aim to contact the referring team to discuss the referral within 24hrs or the next working day.

For Urgent referrals over the weekend please contact the office, being specific regarding the urgency.

Patients can expect to receive specialist advice on symptom management according to the standard.

For community referrals please complete a referral form and the referring team will be contacted within the standard response time.  Please send Monday-Friday via fax 01904 777049

At weekends there is a limited service, any referral taken will need to be accessed through the district nursing team.

Following initial assessment a copy of record of consultation will be offered to the patient.

While contagious viruses are active year-round, winter is a prime time for norovirus, colds, influenza (flu), and other respiratory illnesses.


These infections are highly contagious, so we urge people to stay away unless their visit is absolutely essential to help reduce the spread of infection within our hospitals.

Hospitals are full of sick people, many of them frail and elderly, so bringing germs into a hospital is the worst thing you can do.  We have an active programme of infection prevention but we can’t spot visitors who really shouldn’t be here.  Please stay at home if you’re unwell to help aid the wellbeing and recovery of our patients.
