Celebration of Achievement 2025: Patient Choice Award

The Celebration of Achievement nomination period is open until Thursday 31 July 2025. We welcome nominations from patients, relatives, and visitors for an individual or team at York and Scarborough Teaching Hospitals NHS Foundation Trust.

Every year, we ask patients and their families to share their stories about a local hero - an individual or team who has made a real difference to their experience and left a positive, lasting impression.

This award gives the public the chance to say ‘thank you’ by recognising the outstanding care, treatment and kindness freely given when they needed it most.

Judges will be looking for:

Evidence of outstanding care, treatment and kindness that went above and beyond expectations
A lasting, positive impact on a patient or family member’s experience at a time they needed it most
Personal stories or testimonials that highlight the individual or team’s compassion, dedication and the difference made

What happens next?

We will be in touch to verify your nomination. If your details cannot be verified, your nomination will be withdrawn.

Once your nomination has been successfully verified and we have received all nominations, your nominee will be informed.

The judging period will take place throughout August.

How will we use your nomination?

The Trust uses nominations in a number of ways in order to represent the services we deliver, including in printed information, displays, exhibitions, on our website and on social media.

Successful nominators will also be invited to contribute to a small film about their nominee.
