Community response teams
We currently provide two community response teams to local communities. These include a service in the City of York (which also provides some services in Easingwold), and one in Selby and District.
The City of York service was developed by bringing together the existing Fast Response and Intermediate Care Teams. These teams (made up of nurses, physiotherapists, occupational therapists and generic support workers) are able to support people to achieve short term goals to maximise their independence. This can be to help prevent an admission to hospital or to support an earlier return home following a hospital stay.
The Selby service is similar, and was developed and is run jointly with North Yorkshire County Council. The team includes social care assessment and reablement staff and is able to provide a more comprehensive service.
The teams work from 8am to 8pm, seven days a week - including bank holidays. They develop individual care plans with people they are supporting and their carers. They are able to provide intense support over a short period and work closely with other teams in the community (such as GPs, district nurses, care providers and community and voluntary organisations) to ensure that long term needs are identified and met.

From 1 May 2018 Humber Teaching NHS Foundation Trust took responsibility for the provision of adult community services for Scarborough and Ryedale.
