Heart Failure Specialist nurse team

There are several heart failure specialist nurses based across the Trust.

The specialist heart failure team are involved with the care of patients with heart failure in both the hospital and community settings, from the time of diagnosis of this condition.

There are two full-time and one part-time heart failure specialist nurses covering the community of Scarborough, Whitby and Ryedale and providing in-reach support to Scarborough Hospital.

Services are provided at:

  • Scarborough General Hospital
  • Whitby Community Hospital
  • Malton Community Hospital
  • All GP surgeries in Scarborough, Whitby and Ryedale
  • Out in the community

There are also five heart failure specialist nurses, one full-time and four part-time, covering the community of York and Selby providing support to patients, GPs and York Hospital.

Services are provided at:

  • York Hospital
  • Selby War Memorial Hospital
  • Malton Community Hospital
  • Clinics at GP surgeries; Easingwold, Haxby, Clementhorpe, Clifton and Acomb

Referrals

Referrals are accepted from cardiologists, GPs, district or practice nurses.

Once a referral is received the team will telephone the patient to make the appointment.  Nurses can visit patients in their home or invite them to a clinic setting of their choice.

We are aware of a fire within the grounds of Malton Hospital.

All patients and colleagues have been safely evacuated, and there are no reports of any injuries.
