You can fill in the advance care plan as much, or as little as you like, it is up to you. Please share this plan with those who are important to you, your GP, and any health professionals involved in your care. If you need more space, please continue on plain paper, include your name, and date of the plan on each sheet.
You can either fill this in online and print or save it or you can print off the blank form to fill in by hand. If you have any problems with this form, please contact the Palliative Care Team

If you have travelled from one of the countries or  areas listed on that website and have a fever, shortness of breath or a cough please do not go to the hospital or your GP. Stay at home and call NHS 111 for further advice, even if your symptoms are mild.

If you need to come to hospital for any other reason  please be reassured our hospitals are functioning as normal.
