What to do next

The bereavement offices are open Monday to Friday from 8.30am to 4.30pm.

On the next working day after the death has occurred, please call the Bereavement Services Team after 9am; they will advise you on the next steps of the process.

Please note that the Medical Certificate of Cause of Death (MCCD) will only be available once completed by the doctor. The relevant staff will advise you once this is completed.

Please be aware that on Mondays and Tuesdays after a bank holiday our phones are exceptionally busy. We do have a voicemail answering system so please do leave a message. On particularly busy days there may be a delay in returning your call, but please be reassured that we will be liaising with the ward staff and doctors to arrange for the Medical Certificate of Cause of Death to be completed.

Please note that it can take up to 3-4 working days for the MCCD to be completed. However the bereavement officer will keep you updated if there are going to be any significant delays with the certificate being completed.

You will need this certificate in order to register the death; Bereavement Services will be able to advise you how to do this.  However, if the death has been referred to the coroner there could be a delay in the certificate being issued. The relevant staff will keep you updated about this.

Update for visitors

Visiting restrictions are in place at our hospitals to stop the spread of norovirus. These restrictions will be in place until further notice.

We understand these restrictions can be difficult when a loved one is unwell. The restrictions will be reviewed daily and lifted as soon as it is safe. Thank you for your understanding as we try to reduce the spread of norovirus in our hospitals.
