How to save the Staff Benefits and Wellbeing section to your home screen
iPad or iPhone
- Launch “Safari” app.
- Enter into the address field the URL of the website you want to create a shortcut to. Tap “Go.”
- Tap the icon featuring a right-pointing arrow coming out of a box along the top of the Safari window to open a drop-down menu.
- Tap “Add to Home Screen.” The Add to Home dialog box will appear, with the icon that will be used for this website on the left side of the dialog box.
- Enter the name for the shortcut using the on-screen keyboard and tap “Add.” Safari will close automatically and you will be taken to where the icon is located on your iPad’s desktop.
Android
- Launch “Chrome” app.
- Open this link.
- Tap the menu icon (3 dots in upper right-hand corner) and tap Add to homescreen.
- You’ll be able to enter a name for the shortcut and then Chrome will add it to your home screen.
The Trust is committed to celebrating the lives and work of those who have died while in service, as well as those who dedicated many years of service to the Trust before retiring.
The loss of a staff member, whether current or retired, can be deeply felt by colleagues who built meaningful and lasting relationships with them throughout their career. To honour their memory and support the grieving process for family, friends, and colleagues, the Trust has dedicated memorials at each hospital site where individuals can be remembered and reflected upon.
At the larger hospital sites, these memorials also provide the opportunity for an individual's name to be permanently commemorated. These include the Pillars of Reflection at York Hospital and the Tree of Life at Scarborough and Bridlington Hospitals.
Application form
If you would like to have a name included on the installation at York, Scarborough or Bridlington, please complete the below online form or email yhs-tr.staffbenefits@nhs.net for a printable version.
The form is to be completed by the line manager of the deceased, before completing this request please consider the wishes of the family and any 'known' wishes of the individual.
Please submit this form only once.
For Scarborough and Bridlington memorials, person's name will be presented on a leaf, which can be sent to the line manager to be hung on the installation with an accompanying service if requested.
For York, person's name will be presented on a plaque which will be installed on the pillars, the requesting line manager will be notified once the name has been installed, after which a service can take place if requested.
