Information Governance

Information governance provides a framework to bring together all the legal rules, guidance and best practice that apply to the handling of information.  This includes personal information, like that contained in health records and business information like financial records and policies.

The following pages are part of the Trust’s information governance obligations:

• Privacy notices – how we handle your information
• Patient record requests – to see your health record, or request a record on behalf of someone else, including the deceased, this page will tell you how to make a request
• Data opt out – if you have concerns about your health records being used for research and planning this page will tell you how to stop this
• Freedom of information – to access Trust information that isn’t yours or someone else’s personal information, this page will tell you how to make a request.
